Overview

Home Health by Hale Makua is personalized treatment for medical conditions provided in the comfort of your own home, with the goal of preventing re-hospitalization. Those qualifying for services are functionally or medically homebound, in need of intermittent skilled nursing or therapy services, and have a Hawaii physician overseeing their care.

Service Rates:
Cost will vary depending upon the services you need. Most insurance companies pay for home health care, including Medicare and Medicare HMOs, HMSA, Quest Plans, Medicaid, Workers Compensation, Veterans' Administration, and most mainland insurances. We will help you verify coverage prior to providing services.

Foster Family


Foster family for adults is a program for individuals who want live in a home and remain in the community. Foster families provide a room in their home, meals, personal care, transportation to all appointments, help with medications, and planned outings with family members. The program requires that at least one caregiver be at home to provide care and service at all times. The primary caregiver must be an independent care provider who is certified as a Certified Nurse Assistant, a Licensed Practical Nurse, or a Registered Nurse. All caregivers are screened and certified by the Department of Human Services, and a case manager visits the home monthly to assure quality care for clients and to provide assistance for caregivers. The case management team includes registered nurses and licensed social workers.

The Foster Family program is for individuals who:

  • Prefer to live in a residential setting rather than in a nursing home
  • Are in need of nursing care and have a physical-rated nursing home level of care
  • Do not display violent or abusive behavior, and do not have a drug or alcohol abuse problem

The monthly fee for a Foster Family private room is about half the cost of a semi-private room in a long-term care facility. If the individual qualifies for Medicaid, room, board, and personal care are all covered. Service costs are based on the amount of medical assistance needed.
