Care Management

Kālele Care Services provides kūpuna and their caregivers care management with support, advocacy, and respect through their journey.

Caring for kūpuna (elderly) is one of society's most essential tasks, yet a fragmented health care system misses opportunities to ensure that our aging population is supported across the continuum of care. At Ohana Pacific Health, our aim is to provide kūpuna with care, support, and respect throughout their care journey.

Needs are supported through transition of care support, health education, medical and medication management, and social services assistance using a culturally relevant approach.
