Our long-term care communities are Medicare and Medicaid certified. We accept insurance from Kaiser Permanente, HMSA, Veterans Administration, and most private insurance companies including HMOs.     Physician Referral

    A physician’s referral is necessary for admission to the nursing facility. The physician must agree that placement is appropriate. The resident cannot be admitted based on personal or family choice alone. The referral will include information about primary diagnosis necessitating admission, current functional level, and insurance and/or financial information.

    Financial Eligibility

    Our Admissions team will work individually with each prospective resident to identify the source of payment. You may be asked to disclose information about insurance coverage, income and assets. Care may be covered by a long-term care insurance provider or Medicaid.

    Pre-Admission Evaluation

    A Registered Nurse will conduct an a pre-admission assessment of the resident’s medical condition to determine if the resident meets the required criteria for admission, the suitability of the environment for the resident’s needs, and determine the level of care needed.

Family Resources

Respite Care

Respite care is the ideal option for caregivers needing a break or a chance to travel. Individuals needing assistance with daily activities or requiring nursing care will find our respite program provides a safe, secure environment for a period of 3 to 30 days.

Individuals must be under a physician’s care and need nursing care or supervision to be eligible for services, which include:

  • A private room including a full bathroom
  • Around the clock nursing care and support
  • A personalized plan of care
  • Personalized individual and group activities
  • Laundry and housekeeping services
  • Customized exercise programs
  • Individualized meals and a variety of snacks
  • Skilled therapy as prescribed by a physician
  • Social work services
