Rates

Cost will vary depending upon the services you need. Most insurance companies pay for home health care, including Medicare and Medicare HMOs, HMSA, Quest Plans, Medicaid, Workers Compensation, Veterans' Administration, and most mainland insurances. We will help you verify coverage prior to providing services.

  • Process Slide: Pre-Admission

    Pre-Admission

    A hospital or physician must refer you for skilled home health services. Insurance coverage is verified. A face sheet, recent history, and physician’s orders for need for services must be completed. If accessing your Medicare benefit, your physician must be in the PECOS system and complete a face-to-face certification.

  • Process Slide: Care Coordination

    Care Coordination

    Once the pre-admission forms have been completed and a hospital or physician referral received, your case will be assigned to a Home Health Registered Nurse (RN), Physical Therapist (PT), or Speech Therapist (ST) to best coordinate and follow through with your care based on your physician’s orders and health needs.

  • Process Slide: Home Visit

    Home Visit

    Upon admission, a Home Health Registered Nurse, Physical Therapist, or Speech Therapist will visit you at your home to ensure that the criteria of being a Home Health client are met. To received Home Health services you must be home-bound and require skilled nursing, physical therapy, or speech therapy services.
