Job Description

Carries out effective patient treatment plans to restore, compensate or adapt for loss of patient function, which are established by the licensed/registered therapist. Plans conform to federal, state and professional standards.

Essential Job Duties

  • Visits Home Health patients for implementation of the physical therapy care plan as established by the licensed therapist.  Administers prescribed physical therapy treatment programs. Meets the patient’s goals and needs and provides quality care.  Helps patients accomplish treatment plan and secures necessary supplies and adaptive equipment to facilitate independence in patients

  • Administers therapy treatments according to a treatment plan approved by the attending physician and licensed therapist.  Modifies treatments in conjunction with licensed therapist as necessary to achieve desired outcome

  • Completes an effective discharge plan in conjunction with trans-disciplinary team throughout episode of care. Ensures smooth and timely transition from skilled treatment to outpatient/wellness programs as appropriate for individual patients

  • Communicates treatment plan and progress with patient, family and caregivers in conjunction with therapist

  • Remains knowledgeable of state practice acts and ensures the level of supervision they receive meets standards.  Does not provide treatment outside of practice act limits

  • Documents patient care services by charting in patient and department records according to accepted regulatory, corporate and professional guidelines. Records daily treatment charges per corporate procedures. Documentation will be completed using a company furnished hand held electronic device (i.e. Ipad).

  • Protects patients and employees by adhering to treatment location infection control policies and protocols and keeping information confidential per company and federal requirements.  Maintains safe and clean working environment by complying with treatment location and department procedures, rules and regulations.

  • Provides services in multiple settings (including skilled nursing, assisted living, patient homes, and outpatient) as determined by supervisor and patient/resident need including community based services such as home health and wellness activities as required

  • Travel is required to perform job

Qualifications

  • High school diploma or equivalent, preferred

  • Current licensure as Physical Therapy Assistant in practicing state and/or nationally certified

  • Ability to travel to patient homes within designated geographic area.

  • Valid driver’s license and proof of auto insurance

  • Current CPR certification

  • Must be capable of maintaining regular attendance
